[Booki-dev] Web format PDFs are unusable

Several people have reported that the web format PDFs produced by
OBJAVI are unusable in Windows.  Actually, the problem is that the
table of contents is missing a root object.  If you try to open the
PDF with Adobe Reader it fails with an error message to that effect,
even on Linux.  Other PDF readers which do not support the Table of
Contents (or perhaps do not open it by default) can open the PDF.
