its because you have two chapters with the URL 'introduction' one in the
intro and one in the add-ons section

we have to work around this manually for now - best to copy the contents
of one into another (new) chapter...

in time we will work out a nice way to prevent this kind of thing
happening but for now just dont call two chapters by the same name *when
you create them*...you can always change the names once the chapters are
created to have the same name as this does not effect the URL

the issue may have arisen during import because of the way booki creates
urls at import...so anyone else doing an import should just double check
this after the import is complete. the easiest way to double check is to
do this look at the 'read only' view of the manual and browse your mouse
over the table of contents/index on the left...then look for duplicate
links
