Hi James,
I've temporarily published your project to the front page & here:

the link will stay the same but in the next day or so it should move to
it's more permanent home here:

The title "Make Your Own Sugar Activities!" is different from the
address - but other manuals are like this, so it isn't so much of an
issue. e.g. OX = One Laptop Per Child Users Guide & CircumventionTools =
How to Bypass Internet Censorship.
Regarding the credits, How to Bypass Internet Censorship lists the
authors who initially wrote the manual near the front of the book, maybe
this could work for you too?

Also - if you want to make this book available on Lulu, send me a cover,
or a good image for the cover, and I can upload it for you using the
