Actually, I switched to using 'get booki gears' precisely because it
doesn't do that numbering in the export to FM. Before that, every time I
added a new chapter anywhere except at the end of the book, all the
links to following chapters from third party sites got broken in the
renumbering (which I opened a ticket on booki-dev about).

It's really common for us to need an extra chapter, and it rarely goes
at the end of the book. Writing a manual is not like writing a novel, is
it? :-)
