[FM Discuss] HTML & PDF version of Freedom Fone book not publishing correctly

I'm not sure when the HTML & PDF versions of our book on the 
http://en.flossmanuals.net website stopped publishing correctly, but I 
suspect it happened today when I used bookigears to republish all 
versions of our Freedom Fone book.

I received email notifications that epub, pdf and book versions were 
done, but on checking the updates I noticed:

a) The PDF is not correctly styled and is missing chapter images
b) All chapter links in the HTML version display a message similar to 
the following:

"Error
This page does not exist. Book request=freedom-fone Chapter 
request=ch002_what-does-freedom-fone-do"

Note that the PDF generated from http://booki.flossmanuals.net publishes 
correctly.

Looking forward to some assistance.
