choose advanced and change the margins

so, if you need to add a cover you need to either do it with some css 
wizardry like :after psuedo elements, add it manually with something 
like pdfmod (very good) or wait a few weeks for some magical updates to 
booktype

the rest of what you describe is booklet formatting and thats 
trickier..you might wanna look at this:
