Click 'bookinfo' in the left side navigation menu. You should see a page
with details for your book(s). 'Visible' is the last checkbox in each
book section.

Anyway, in this case the 'Visible' checkbox does not seem to have been
the problem. Adam posted that you might have some objects extending
beyond the canvas. If max-width: 500px works for A5 but 700px doesn't,
it might suggest that you have some images in the book wider than the
recommended maximum of 600 pixels.
