http://www.lulu.com/shop/search.ep?contributorId=1099565 lists only one
book by 'FLOSS Manuals' which seems to be the one you want to remove. I
would suggest contacting http://www.lulu.com/support/ and asking them
for a password reminder to be sent to the email address associated with
that publisher account.
