As it happens, I had already written code to parse an ncx <pagelist>,
which I'd prefixed with this comment:

# Try adding these bits, even though noone has them and they are no use.

So that has maybe turned out for the best.

And, yes, this is the tricky part.  I had a bit of a chat to Adam and
Aco, and none of us had a strong idea of how to track page numbers in a
document that is being edited.
