I don't believe I ever use   in a book when I'm writing in DocBook. (And yes -- he said through clenched teeth -- I've often had to code up DocBook manually. Watch me create a table from scratch some time.)

I noticed only problem in the DocBook you created: extraneous newlines a couple places. For instance, a bold word may have a line break between the word and the following punctuation character. That doesn't look like something specified in the XSLT; it's probably an artifact of the processing tool.
