[FM Discuss] Creating table of contents via anchor links

thanks heaps james for all these bug/issue reports. its *great* to get them.

on  a related issue - i was wondering if you would have a moment to test 
the web-based pdf generation through Objavi with different css, we can 
then decide on a better format?
