[FM Discuss] Content items as clickable links

Hi everyone

Is it possible to make Content Items clickable links to assist people 
with navigating soft copies of books?

In the past I'm sure some versions of our book have opened in Acrobat 
Reader with a Contents sidebar on the LHS but I cannot replicate this 
behaviour any longer.
